Nurse Care Management
When the needs are ongoing and the picture keeps getting more complicated, you should not have to be the project manager of your loved one’s care. We step into that role with you, making sense of it, coordinating it, and bringing in the right people, and you stay informed and part of every decision.
One team that sees the whole picture
Led by a board-certified gerontological nurse
With your family the whole way

Who This Is For
When a loved one’s care has too many moving parts
Most families reach us when those parts start working against each other.
Nurse care management helps with
Multiple doctors who do not talk to each other
A serious or progressing diagnosis
Frequent trips to the hospital
Safety worries at home
The growing sense that you do not have the capacity or knowledge to hold all of this together
What You Can Expect
What changes when a nurse-led team keeps track of everything
One steady team that knows your story
One nurse-led team holds the whole picture, so you stop repeating the story to every provider.
Answers you can act on
We explain what the doctors said and what it means for daily life, then tell you what to do next.
Faster responses when health changes
When something shifts, we are already watching, and we move quickly so small problems stay small.
How It Works
We start by getting to know your loved one
Their story, what matters to them, and what stands in the way. The process is the same for every family; the plan is built for yours.
01
Assessment and understanding
We come to the home and talk through health, memory, medications, safety, and daily routines, at a pace your loved one is comfortable with. A nurse also notices the small things that never make it into a chart.
02
Your plan
We agree with your family on the goals, who does what, and what we are watching for. Your loved one’s values and needs shape it, and it changes as life changes. A formal written plan of care is available as a separate charge. If you would like one, ask us at the start.
03
Coordination and advocacy
We put the plan in motion. We connect you with vetted providers, help you prepare for medical appointments, translate what the doctors say, and respond quickly when something shifts.
04
Ongoing evaluation
We stay involved, watching for what is changing and adjusting the plan so nothing falls through the cracks.
What families say
"Their RN team was on call and monitored my parents’ care through visits and chart reviews. They handled the emergencies as well. As a result, I kept my overseas job and my mental and physical health, brought my parents’ finances under control, and slept well at night knowing that they were comfortable and safe."

Questions families ask us
You call, we listen, and we talk honestly about whether we are the right fit or whether someone else would serve you better.
If we are, we send a welcome letter and a short list of what to have ready: current medications and diagnoses, any legal paperwork already in place, a go-bag for an unexpected hospital trip, and practical details like a disabled parking pass. That way your first visit is spent on your loved one, not on forms.
Start with a Care ConsultationNo. We coordinate and oversee care rather than provide it. When hands-on caregivers are needed, we help you find and vet trustworthy ones.
As often as the situation calls for, and right away when something changes. We keep your whole family team in the loop, including the family members who live far away.
See how we support families from a distanceNo. Some families work with us for years. Others just need one good conversation and a road map they can carry out themselves, then check back in a few months later. We meet your family where you are.
Yes. We help families find the benefits your loved one may qualify for and put them to use, including long-term care insurance that often goes unclaimed because no one explained how to claim it. When a special needs trust is involved, we can also petition to release funds for medical and educational needs.
Still have questions? Book a Care Consultation and we will talk through your family’s situation.
